Personal data processing policy

In compliance with Colombian Law 1581 of 2012, Decree 1377 of 2013 and related regulations, Dr. Catalina Moreno Quijano (the "data controller") publishes the following personal data processing policy applicable to those who interact with this website, the office or any associated contact channel.

- Handle medical visit requests and scheduling.
- Provide clinical care and treatment follow-up.
- Send relevant clinical information to the patient.
- Comply with legal obligations regarding medical records (Resolution 1995 of 1999).
- Maintain administrative communication related to the consultation.

- Full name
- Email address
- Phone / WhatsApp
- Patient's age (when applicable)
- Service of interest and preferred modality
- Free-form message describing the reason for contact
Sensitive data: clinical information shared by the patient during consultation is considered sensitive data and is subject to the special provisions of Law 1581.
4. Data subject rights
- Know, update and rectify your personal data.
- Request proof of authorization granted.
- Be informed about the use given to your data.
- File complaints with the Superintendence of Industry and Commerce (SIC).
- Revoke authorization and/or request deletion when applicable.
- Free access to data being processed.
5. How to exercise your rights
To exercise any right described above, send a request to dracatalinamorenoquijano@gmail.com indicating the right you wish to exercise and attaching a copy of your ID. Response will be given within the timeframes set by Law 1581.
6. Validity
This policy is effective from publication. Personal data is retained only for the time needed to fulfill the described purposes and applicable legal obligations, particularly those related to medical record retention.
7. Modifications
The data controller may modify this policy. Changes will be published on this same page with the update date.
